We're Here When You Need Us

Whenever you, or a loved one need nursing help, we are here. CareOne's office is open seven days a week, 24 hours a day. As part of our service, our RN Case Manager makes an initial assessment, follow-up visits, monitors the patient's progress, and notifies the physician of any important changes in his or her condition. To make things easier for you, we can, in many instances, bill the insurance company for you. You'll be reassured to know that all our employees and independent contractors are carefully screened, bonded, and insured.

Our Home Health Care Team Includes:

  • Skilled Nurses
    A full range of nursing services including wound care, IV therapy, diabetic care, catheter care, cardiac and respiratory assessment, medication management, pain management and cancer care.
  • Therapists
    Assessment and treatment by physical therapists, occupational therapists and speech language pathologists for adults and children.
  • Medical Social Workers
    Case management, grief counseling, coordination of community resources, and assistance with social / emotional effects of acute and chronic illness.
  • Home Health Aides
    Assistance with daily living activities and personal care.

Rehabilitation Services

  • Physical Therapy
    Our Physical Therapists assess and provide treatment to help patients of all ages increase mobility, strength, balance, and coordination. Therapists also work with family and community resources to maximize the patient's rehabilitation process.
  • Occupational Therapy
    Occupational therapists assess and treat patients in order to increase their self-help and homemaking skills. Therapists help those who have suffered physical impairment to return to their maximal level of independence.
  • Speech-language Therapy
    Our Speech-language Therapists help patients with their language and speech skills so they may regain control of their communiocation skills. They also teach patients and family members safe and appropriate swallowing techniques.

We believe that every person in our care is a unique individual, to be treated with utmost respect and kindness. Before assigning professional nursing care to your loved one, our Case Manager first makes a home visit to evaluate the patient's needs. A personalized care plan is then prepared at no charge. This incorporates the physician's orders and the special needs of each individual patient. By matching the patient's needs to the appropriate level of care, unnecessary costs are eliminated, and the patient gets precisely the care they require.
